License Restoration Info
License Restoration Information Following Forfeiture
- Forfeited MDG licenses may be restored within 1 year and subject to meeting restoration requirements. Failure to restore a forfeited license within 1 year
after the license expiration date will result in the termination of the license and the license holder must apply as a new applicant and meet current
licensing requirements. - Forfeited EMT, DOS, MD, PO, and AMD licenses may be restored within 2 years and subject to meeting restoration requirements. Failure to restore a
forfeited license within 2 years after the license expiration date will result in the termination of the license and the license holder must apply as a new
applicant and meet current licensing requirements. - To restore a forfeited license, please request a restoration form. You will need to submit the completed form with a wet signature, the restoration fee,
and documentation to verify compliance with continuing education requirements during the previous biennial renewal cycle.
