Insurance Company Corporate Amendments

Name changes, adding and deleting a class of insurance, redomestications, mergers, etc.

Licensed insurance companies submitting corporate amendments must use the Uniform Corporate Amendment Application Forms located at the Uniform Certificate of Authority Application (“UCAA”) website.
