Renewal

Pursuant to Section 436B-14.5, HRS , any license held by a member of the armed forces, National Guard, or a reserve component that expires, is forfeited, or deemed delinquent while the member is on active duty and deployed during a state or national crisis shall be restored if certain restoration requirements are met. Medical (Physician, Podiatrist, Physician Assistant, EMT-B, EMT-P)

Biennial January 31 every even numbered year

On-time renewals for the licensing period of February 1, 2016 to January 31, 2018 began on November 9, 2015 and ended on January 31, 2016

Failure to timely renew will result in the forfeiture of the license. A forfeited license may be restored in two years and subject to meeting restoration requirements. Failure to restore a forfeited license in two years will result in the termination of the license and the person must apply as a new applicant and meet current licensing requirements.

Osteopathy

Biennial June 30 every even numbered year.

On-time renewals for the licensing period of July 1, 2016 to June 30, 2018 begins on Monday, May 9, 2016 and ends midnight (HST) Thursday, June 30, 2016

Renewal hardcopy applications for Osteopathic Physician (DOS) license types will be mailed out May 6, 2016

All online renewals will need to be submitted through “MyPVL” at https://pvl.ehawaii.gov/mypvl

Renewals should be submitted by May 31, 2016

Osteopathic Physicians (DOS) are required to have completed the required CME requirement as contained in HRS §453-6 and Subchapter 5 of the Board’s rules.  The Board may also conduct a random audit to determine whether the CME requirement has been met.
