- All licenses regardless of issuance date, are subject to renewal triennial by December 31, every three (3) years, (2022, 2025, 2028)
On-time renewals for the licensing period of January 1, 2023 to December 31, 2025 (Licensees selected for audit must renew by hardcopy only) |

Restoration Information<br />
(Restore within 1 year after license expiration)
Forfeited licenses may be restored within 1 year and subject to meeting restoration requirements. (Must restore by hardcopy only) |
Failure to restore a forfeited license within 1 year after the license expiration date will result in the termination of the license and the person must apply as a new applicant and meet current licensing requirements. |
