If you held a HI Nonresident Managing General Agent license that was inactive for more than two years.

Nonresident Managing General Agent – Individual (License Reactivation)

If you held a HI Nonresident Managing General Agent license that was inactivated within two years.

Nonresident Managing General Agent – Business Entity (Initial License)

If you held a HI Nonresident Managing General Agent license that was inactive for more than two years.

Nonresident Managing General Agent – Business Entity (License Reactivation)

If you held a HI Nonresident Managing General Agent license that was inactivated within two years.

Nonresident Managing General Agent – Individual or Business Entity (License Reinstatement)

If you held a HI Nonresident Managing General Agent license that has been inactive for not more than thirty days, due to nonrenewal.

Before submitting your application:

You must be licensed as a HI Nonresident Producer, or submitting an application for a HI Nonresident Producer with your Nonresident Managing General Agent license application. Each application must have its own check.

A complete paper application includes:

  1. Form HPL – Hawaii Application for Individual Insurance License.
  2. Licensing Fees. Make check payable to DEPARTMENT OF COMMERCE AND CONSUMER AFFAIRS.

Appointments:

Form APPT – Notice of New Appointment.

An appointment is not required to be submitted at time of application, but an appointment must be in place to do business.

Nonresident Managing General Agent – Individual (License Reactivation)

Before submitting your application:

You must be licensed as a HI Nonresident Producer, or submitting an application for a HI Nonresident Producer with your Nonresident Managing General Agent license application. Each application must have its own check.

  2. Licensing Fees. Contact the Hawaii Insurance Division at 808-586-2788 to verify the applicable fees. Make check payable to DEPARTMENT OF COMMERCE AND CONSUMER AFFAIRS.

Appointments:

Form APPT – Notice of New Appointment.

An appointment is not required to be submitted at time of application, but an appointment must be in place to do business.

Nonresident Managing General Agent – Business Entity (Initial License)

You must be licensed as a HI Nonresident Producer, or submitting an application for a HI Nonresident Producer with your Nonresident Managing General Agent license application. Each application must have its own check.

  1. Form BEPL – Hawaii Application for Business Entity Insurance License / Registration. Submit one with original signature.
  2. The name of a licensed HI Producer (resident or nonresident) who is a natural person that will be responsible for the agency’s compliance with Hawaii insurance laws and rules.
  3. Licensing Fees. Make check payable to DEPARTMENT OF COMMERCE AND CONSUMER AFFAIRS.

Appointments:

Form APPTMGA – Notice of New Appointment.

Insurer must provide a statement of the duties the MGA will be performing on their behalf. An appointment is not required to be submitted at time of application, but an appointment must be in place to do business.
