Life Insurance Policy Illustrations

Act 252, SLH 2000 also mandates certain standards governing the conduct of insurers and producers with respect to the use of life insurance policy illustrations. These standards are codified as Part IV of Article 431:10D, HRS, and are effective July 1, 2001.

Among other things, Part IV requires insurers to notify the Insurance Commissioner whether a life policy is to be marketed with or without an illustration. The form below has been designated as the form to be used for notification purposes. Please refer to Commissioner’s Memorandum 2001-7A .
