Dentist

Applicants to apply to PVL after passing the ADEX Dental Examination.

  • Only the ADEX exam is accepted for licensure.  The ADEX exam is offered by the Commission on Dental Competency Assessments (“CDCA”) and by the Council of Interstate Testing Agencies, Inc.   • The examination located in Hawaii is handled by CDCA.
  • Contact CDCA and CITA for deadlines.
  • After passing the ADEX Dental Examination, file an application for licensure with the Board of Dental Examiners (“Board”) by mailing it to P.O. Box 3469, Honolulu, Hawaii 96801, and request the testing agency send your examination result directly to our office.

 

Dental Hygienist

Applicants to apply to PVL after passing the Hygiene Examination.

  • The CRDTS, WREB, CDCA, and SRTA Dental Hygiene examinations are accepted for licensure.  For more information, visit the Central Regional Dental Testing Services, Inc. (“CRDTS”) at www.crdts.org or call (785) 273-0380; the Western Regional Examining Board (“WREB”) at www.wreb.org or call (602) 944-3315; the Commission on Dental Competency Assessments (“CDCA” – see above); or the Southern Regional Testing Agency, Inc. (“SRTA”) at www.srta.org or call (757) 318-9082.
  • The examination located in Hawaii is handled by CRDTS.

  • NOTE: To qualify for licensure in Hawaii, an applicant must be certified in the administration of intra-oral infiltration and block anesthesia.
