On-time renewals for the licensing period of January 1, 2018 to December 31, 2019 begins on Monday, November 6, 2017 and ends December 31, 2017
Renewal Forms for Naturopathic Physician (ND) will be mailed out November 3, 2017.
Beginning with this 2017 renewal:
In order to renew, ND’s must submit the renewal form, fees and documentation of having met the continuing education (CE) requirements. See Continuing Education Information for more information.
To better ensure receipt of a new renewal pocket ID card by December 31, 2017. Renewals should be submitted by November 30, 2017
Failure to timely renew by the renewal date will result in the forfeiture of the license. After the renewal date, the forfeited license may be restored subject to meeting restoration requirements.
Pursuant to Section 436B-14.5, HRS , any license held by a member of the armed forces, National Guard, or a reserve component that expires, is forfeited, or deemed delinquent while the member is on active duty and deployed during a state or national crisis shall be restored if certain restoration requirements are met.
