PEST CONTROL OPERATOR RENEWAL REQUIREMENTS (for the licensing period of July 1, 2020 to June 30, 2022) |
FIRMs and SOLE PROPRIETORs (Active Status) Must be current with the below at the time of renewal:
Liability insurance, valid thru July 1, 2020 |
Workers compensation insurance, valid thru July 1, 2020 (If no employees-must have filed exclusion from chapter 386, HRS form) |
Tax clearance or tax compliance letter from the Hawaii Department of Taxation, no older than 6 months of July 1, 2020 |
Designated Principal RME (All RMEs must also be renewed before license will be renewed.) |
SOLE PROPRIETOR and RMEs (Active Status) Must submit proof of valid, current certification with the Department of Agriculture in the appropriate category and subcategories (section 16-94-40, HAR) with your renewal.
